Community Support Services

Friendly Visiting | Telephone Assurance Program
Footcare | E.S.C.O.R.T. Transportation Program

Community Support Programs provide services that promote independence for the older adult especially those who are homebound and frail.

Included in these services are a Friendly Visiting Program, Telephone Assurance, Basic and Advanced Footcare, Transportation, Information and Referral, Educational Sessions, Income Tax Clinics and Health Counselling by Registered Nurses.

Friendly Visiting
Older adults are visited by caring volunteers on a regular basis. If you would like to be matched with or be a Friendly Visitor, please contact the Community Support Co-ordinator at (519)254-1108.

Telephone Assurance Program
T.A.P is a service providing assurance through regular phone calls. Participants make a commitment to make or receive one phone call each day. Contact the Community Support Coordinator at (519)245-1108 to register for this free service.

Footcare
Basic footcare is provided by Professional Nurses with Basic and Advanced Foot Care Certification who bathe the feet, trim nails and treat corns and calluses. Footcare Clinics are offered at McEwan Avenue Centre, Edward Street Centre, St. John's Parish Hall and the Jewish Community Centre.

For the first appointment, the fee of $21 for Members and $26 for Non-Members includes a thorough assessment of the individual's feet and general health. Then appointments are $16 for Members and $22 for Non-Members.

**Footcare is also provided at Central Park Lodge, Huron Lodge Home for the Aged, Oak Park Lakeshore and Sunrise Assisted Living for a flat fee.

Transportation - E.S.C.O.R.T.
This service provides transportation to medical appointments, banking, shopping, social activities and more. Advance booking is required. There are two options:

Agency Van: a door-to-door service for those able to board a van with minimal assistance.

Individual volunteer drivers use their own vehicles to transport individuals who require some assistance. The driver will accompany the older adult into their appointment and see that they return home safely.

When booking a ride, please have the following information ready:

your name and phone number
date and time of your appointment
address to be picked up at and dropped off at
expected return time
state whether you will require an individual driver or if you can use the agency van.

Eligibility Criteria for Van Riders
The E.S.C.O.R.T. service is available for those who meet the following eligibility criteria:

are unable to access public transportation because of short or long-term physical limitations such as heart disease, arthritis, Alzheimer disease and other cognitive impairments
require escorts during the trip because of frailty or cognitive impairment
specialized transit is not available or the individual does not meet the eligibility criteria
public transportation is not available and friends and relatives are not available or able to provide transportation

Client Fee

$6 one way; $12 round trip
